Consulting Agreement

I agree to use the best of my abilities and unique gifts to help you make the changes you seek, and I agree to strict confidentiality. I am professionally committed to using my skills and gifts to assist you in mobilizing your own resources to achieve the results you seek.

From you, the Client:

I understand that my success depends on my own commitment to improving my situation.I acknowledge that Kimberly Wilborn is not diagnosing or prescribing medical treatment, and I do not hold her responsible for physical or mental ailments. I release Kimberly Wilborn from any liability whatsoever regarding my session. Any conclusion I draw from my session or responses I have are mine and mine alone. I take full and total responsibility. I understand that these sessions are not psychotherapy, but are an alternative aimed at creating positive changes in my life.

I understand and believe that self-improvement requires commitment on my part and I must be willing to change in a positive way if I am to receive the full benefit of Kimberly Wilborn's work.

I understand the above, and I take full responsibility for myself.
